CFC is a Medicaid entitlement program — no waitlist. Built for individuals who require a nursing facility
level of care but choose to remain at home. It explicitly allows spouses and adult children to serve as paid
caregivers through self-direction.
Community Personal Assistance Services serves individuals who need help with at least one ADL but do
not meet the nursing facility threshold required for CFC. No waitlist. Self-directed care including paid
family caregivers is available.
Yes. Maryland’s Community First Choice program explicitly allows spouses to serve as paid caregivers
through self-direction, provided the spouse is not also the participant’s legal guardian. This is more
permissive than most states, including Michigan.
No. Both CFC and CPAS operate as entitlement or state plan benefits — anyone who meets the eligibility
criteria receives services. There is no enrollment cap or waitlist for either program.
